The scope of personal information that our company obtains from you is information regarding identification as described in (1) to (6) below.

2. Documents to be submitted for "Requests for Disclosure, etc."

In the case of "Requests for Disclosure, etc.," please fill in all the required items on the written request forms in (1) and send them by mail. Regarding the written request forms, we will send the following forms by mail. In order to verify your identity, you will need to enclose copies of identification documents such as your driver's license and passport.

(1) and the identification documents in A, the agent's identification documents will be required in accordance with the method described in A.
    4. When a legal representative makes a request for disclosure, etc., a copy of a document showing the agency relationship between the principal and the legal representative, such as a copy of the family register, a certificate of adult guardianship registration, etc., is required. When a voluntarily appointed agent makes a request for disclosure, etc., the original of a document showing the agency relationship between the principal and the discretionary agent, such as proxy, etc. is required. Fee for "Request for Disclosure, etc." and method of collecting it
  1. In principle, there will be no fee for requesting disclosure or notification of the purpose of use.
  2. Actual expenses may be charged. If we charge an amount equivalent to the actual cost, you can pay it by mailing a money order, postage stamps, etc., and the person making the request will be responsible for the mailing cost.
How we will answer a "Request for Disclosure, etc."

We will generally respond in writing.

  1. Personal information obtained in connection with the "Request for Disclosure, etc." shall be handled only to the extent necessary to respond to the request.
  2. We may not be able to respond to "Request for Disclosure, etc." in the following cases. In that case, we will inform you of that and of the reason. Note that we may charge a fee even in cases of nondisclosure.
    • When the identity of the person in question or his/her agent cannot be confirmed
    • When there are mistakes in the prescribed request procedure
    • When the subject of a request for disclosure, etc., does not fall under "Personal Information Subject to Disclosure" *
    • When there is a risk of harming the life, body, property or other rights or interests of the person in question or a third party
    • When there is a risk of causing considerable impediments to the proper implementation of the business of our company
    • In cases that constitute a violation of laws and regulations
